Epididymo-Orchitis: Causes, Symptoms, Diagnosis, Treatment and Recovery

Sudden pain and swelling in the scrotum is alarming for any man. One of the most common causes is epididymo-orchitis - an inflammation of the epididymis (the coiled tube behind the testis that stores sperm) and the testis itself. It affects men of all ages, from teenagers to the elderly. The good news is that with early diagnosis and the right antibiotics, almost all men recover fully. What is Epididymo-Orchitis?
Epididymitis is inflammation of the epididymis, and orchitis is inflammation of the testis. Because the two lie so close together, infection very often spreads from one to the other, and the condition is then called epididymo-orchitis. It can be acute (symptoms for less than 6 weeks, usually painful and sudden) or chronic (pain or discomfort lasting more than 3 months).
Causes of Epididymo-Orchitis
1. Sexually transmitted infections (STIs)
In sexually active men under 35, the most common causes are Chlamydia trachomatis and Neisseria gonorrhoeae. Infection travels from the urethra down the vas deferens to the epididymis.
2. Urinary tract infections
In men over 35, and in men who have anal intercourse, bacteria from the urinary tract such as E. coli are the usual cause. Risk is higher with an enlarged prostate, urethral stricture, recent catheterisation, cystoscopy or prostate surgery.
3. Viral infection - mumps orchitis
Mumps can cause painful orchitis, typically 4-7 days after swelling of the salivary glands, especially in teenagers and young men who are not vaccinated.
4. Tuberculosis and other infections
In countries where TB is common, genito-urinary tuberculosis can cause a slow, painless or mildly painful swelling of the epididymis, sometimes with a discharging sinus in the scrotum. Brucellosis and fungal infections are rarer causes.
5. Non-infectious causes
Certain medicines (such as amiodarone), injury to the scrotum, heavy lifting with reflux of urine into the vas deferens, and autoimmune conditions such as Behçet's disease can also cause inflammation.
Symptoms
- Pain and swelling on one side of the scrotum, developing over hours to a few days
- Redness and warmth of the scrotal skin
- Tenderness of the epididymis and testis; pain may spread to the groin or lower abdomen
- Fever and chills in more severe infection
- Burning while passing urine, frequent urination or discharge from the penis
- Pain during ejaculation or blood in the semen
- A hard, tender lump behind the testis
Warning: Sudden, severe testicular pain - especially in boys and young men - can be testicular torsion, a surgical emergency where the testis twists and loses its blood supply. Torsion must be ruled out immediately, as the testis can be saved only within about 6 hours.
How is Epididymo-Orchitis Diagnosed?
- Clinical examination by a urologist - checking the position of the testis, tenderness and the cremasteric reflex
- Urine routine and culture to identify bacteria and choose the right antibiotic
- First-void urine NAAT (PCR) test for Chlamydia and Gonorrhoea, and a urethral swab if there is discharge
- Scrotal ultrasound with colour Doppler - shows increased blood flow in infection, rules out torsion, and detects abscess or tumour
- Blood tests - CBC, CRP, blood sugar; blood cultures in severe cases
- TB tests (urine AFB / GeneXpert) in chronic or non-healing cases
- Tumour markers (AFP, beta-HCG, LDH) if a solid mass is seen, because testicular cancer can sometimes mimic infection
Treatment of Epididymo-Orchitis
Antibiotics
Treatment is started immediately, based on the most likely cause, and adjusted when culture reports arrive:
- Likely STI cause: a single injection of ceftriaxone plus a course of doxycycline for 10-14 days
- Likely urinary (enteric) cause: a fluoroquinolone such as levofloxacin or ofloxacin for 10-14 days, or as per culture
- Sexual partners should be tested and treated when an STI is the cause, and intercourse avoided until treatment is complete
- Tuberculous epididymitis needs a full course of anti-TB treatment for 6 months or longer
Supportive care
- Rest and scrotal elevation or a supportive underwear / scrotal support
- Ice packs for 15-20 minutes several times a day
- Pain-killers and anti-inflammatory medicines
- Plenty of fluids and treating any urinary blockage (e.g. enlarged prostate)
When is hospital admission or surgery needed?
Most men are treated as outpatients. Hospital admission and intravenous antibiotics are needed for high fever, severe pain, diabetes or poor immunity. Surgery is required when:
- An abscess forms - treated by drainage (epididymotomy)
- Infection or pain persists despite treatment - removal of the epididymis (epididymectomy)
- The testis is destroyed by severe infection, abscess or infarction - removal of the testis (orchiectomy)
- Torsion cannot be excluded - urgent scrotal exploration

Is epididymo-orchitis contagious?
The condition itself is not contagious, but if it is caused by a sexually transmitted infection, the infection can be passed to sexual partners. Partners should be tested and treated.
Can epididymo-orchitis cause infertility?
Most men recover without any effect on fertility. Severe, recurrent or bilateral infection can scar the epididymis or damage the testis and reduce fertility, so early and complete treatment is important.
How long does epididymo-orchitis last?
Pain improves within a few days of antibiotics, but complete resolution of swelling may take several weeks. Chronic epididymitis can last more than 3 months.
How can I tell epididymo-orchitis from testicular torsion?
Torsion usually causes sudden, very severe pain, often with nausea and vomiting, in boys and young men, and the testis may lie higher than normal. Epididymo-orchitis develops more gradually and is often associated with urinary symptoms or fever. Only an urgent examination and Doppler ultrasound can tell them apart - never wait at home with sudden severe testicular pain.
Do I need surgery for epididymo-orchitis?
Usually not. Surgery is needed only for complications such as an abscess, an infection that does not respond to antibiotics, a damaged testis or uncertainty about torsion.
